Happy Tails Owner Agreement

I certify that my dog is in good health and has not been ill with any communicable condition in the last 30 days. I further certify that my dog has not harmed or shown aggression or threatening behavior toward any person or any other dog. I have read and understand the following.

1. I understand that I am solely responsible for any harm caused by my dog while my dog is attending Happy Tails Doggie Daycare.
2. I understand and agree that by enrolling my dog in Happy Tails Doggie Daycare the staff have relied on my representation that my dog is in good health and has not harmed or shown aggression or threatening behavior toward any person or any other dog.
3. I understand and agree that Happy Tails and Happy Tails staff will not be liable for any problems which develop provided that reasonable care and precautions are followed and I hereby release them of any liability of any kind arising from my dog’s participations and attendance at daycare.
4. I further understand and agree that any problems which develop with my dog will be treated as deemed best by the staff of Happy Tails at their sole discretion and that I assume full financial responsibility for any and all expenses involved.

I certify that I have read and understand the policies of Happy Tails Doggie Daycare as set forth on the preceding pages and that I understand and have read the conditions and statements of this agreement, including the following:

Fees: Fees are bases on a prepaid pass plan. A pass is valid for 60 days from the date of purchase.
Days and hours of operation: Monday through Friday 7:00 a.m. to 6:00 p.m. There will be a $1.00 per minute late fee for any dog that is not picked up by 6:00 p.m.
